Eligibility for Services from Muscular Dystrophy Queensland

Muscular Dystrophy Queensland is committed to providing services based on a high level of specialised knowledge of inherited neuromuscular conditions so Queenslanders with a diagnosis of muscular dystrophy or other neuromuscular conditions are welcomed as clients.

To be eligible to receive services offered by Muscular Dystrophy Queensland the person needs to:

  • have evidence of diagnosis of muscular dystrophy or other neuromuscular condition

and

  • live in Queensland (some conditions may apply).

To become a client, you will need to complete a MDQ Registration Form and Consent Form.  MDQ strongly recommends that clients also register with Disability Services (DS) and provide consent for our staff to share information with DS for the purpose of providing services.

Muscular Dystrophy’s Queensland’s Services

The National Disability Insurance Scheme (NDIS) is progressively being introduced around Queensland and Australia. Though the NDIS may change the way you receive some of the services provided by us, MDQ is committed to continuing to provide help for our families.

Information and Support

  • Helpline – this is a phone service, available to all members of the Queensland community with a need for information about all aspects of any neuromuscular condition.
  • Information through MDQ website (www.mdqld.org.au), factsheets, updates and social media.

Case Management

MDQ’s Case Management Officers (CMOs) work closely with clients and family members to:

  • Understand the client’s current services and needs
  • Identify gaps and help set goals for positive change
  • Make a plan
  • Make necessary changes to their lives according to their plan
  • Regularly review and support client and family throughout their lives.

No matter where the client lives in Queensland, there will be a Case Management Officer assigned to him or her. Our CMOs provide information and support to clients throughout their lives and especially at diagnosis, life transitions, and progressions in their condition.

 

Some of the tasks the CMOs may assist with include:

  • Individual Advocacy which is an integral part of MDQ’s case management service. We assist the client to have a voice and we also help to negotiate a better deal in individual situations.
  • Liaising with the support provider to obtain or adjust services
  • Researching the participants’ local community for leisure activities
  • Advising the client to seek specialist assessments (eg Occupational Therapy or Physiotherapy) and assisting him to find a suitable service
  • Assisting in updating diagnosis and referral for medical treatment
  • Providing information and making appropriate referrals for equipment, home modifications and housing.

The introduction of the NDIS will change the way we are able to provide Case Management services. If you are transitioning to the NDIS please talk to you CMO about how to add Coordination of Supports from MDQ into your NDIS plan.

 

Your Life Your Choice (YLYC)

is currently available to clients with individualised funding packages who choose MDQ as their YLYC Host Provider. Through Pathway Planning at six monthly intervals our YLYC Coordinator enables the person to take control of their funding to meet their needs and to focus on selfmanagement – a useful skill to develop heading into the NDIS.

 

Equipment Assistance (these services are limited and there is an application process)

At the moment, MDQ aims to assist in funding the following equipment for our clients:

  • Hi-lo beds for long term loan, especially to clients who are not eligible for an NDIS Individual Funding package.
  • Rental of cough-assist machines.

Many equipment needs will potentially be covered by the NDIS. It is important that you have a discussion with your CMO before transitioning to the NDIS if you are using equipment from MDQ. If you have a bed from MDQ it is important that you contact us BEFORE you go into your planning meeting with the NDIS. We will need to discuss with you the ongoing use of the bed and any future maintenance or repair costs as this may be something that can be claimed through the NDIS.

 

Financial Assistance for Clients (Conditions apply for this program)

  • No Interest Loans Scheme (NILS): This client program is an interest free loan of between $200 and $2000 for clients experiencing financial hardship. NILS can be used, for example to purchase medical equipment, household appliances or even a holiday, however the scheme does not provide for recurring living expenses such as rent.

 

Supporting better health for people with neuromuscular conditions

MDQ aims to assist clients to lead healthier lives in their community and to minimise periods of hospitalisation by supporting preventative programs. We do this through:

  • Supporting parents and children when they attend the paediatric neuromuscular clinic at the Lady Cilento Hospital in Brisbane
  • Providing rental costs for cough assist machines
  • Educating health professionals and educators working with people with inherited neuromuscular conditions.

Muscular Dystrophy Queensland and the NDIS

Muscular Dystrophy Queensland is playing an integral role in our clients’ preparation for the National Disability Insurance Scheme (NDIS) including:

  • Providing NDIS Readiness workshops throughout Queensland aimed at providing information about the NDIS targeted to clients and families living with inherited neuromuscular conditions
  • Providing NDIS information to individual clients and families
  • Assisting clients in all possible ways to prepare for their NDIS planning meeting
  • Accompanying clients to their NDIS Planning meetings where practicable.

MDQ’s service structure under the NDIS

 

Initially, Muscular Dystrophy Queensland is intending to provide the following NDIS services under the support category Improved Life Choices:

  • Plan Management Provider
  • Coordination of Supports

PLAN MANAGEMENT PROVIDER (similar to the Your Life Your Choice YLYC program): When a person becomes an NDIS participant, they are responsible for organising suppliers of services, paying service providers and submitting reports to the National Disability Insurance Agency (NDIA). This can be very daunting, especially in the early years. As a Plan Management Provider, MDQ can assist our clients with the following important tasks, as an intermediary (explanation below):

The NDIS aims for you to work towards self-managing your NDIS plan so MDQ will provide education sessions aimed at increasing your independence in managing your Plan.

 

Benefits of using Muscular Dystrophy Queensland as Plan Management Provider:

  • Muscular Dystrophy Queensland has the expertise to take responsibility for all financial tasks and to get maximum benefit from the NDIS plan
  • The client retains a relationship with their Case Management Officer from MDQ and continues to benefit from the organisation’s specialist expertise.

COORDINATION OF SUPPORTS (similar to MDQ’s current Case Management)

MDQ will provide the following Support Coordination options:

  • Provide immediate assistance during significant changes in circumstances e.g. a progression in muscle weakness leading to higher support needs
  • Coordinate funded service providers from a range of sources

 

Benefits of using Muscular Dystrophy Queensland as Plan Management Provider

  • Muscular Dystrophy Queensland has the expertise about all aspects of neuromuscular conditions as well as in-depth knowledge of networks and services/service providers
  • The client retains a relationship with their Case Management Officer from MDQ and will continue to receive case management supports.

Please be aware that if you are eligible for an NDIS Individual Funding Package and you wish to continue to receive Case Management Services from Muscular Dystrophy Queensland, you will need to ask the NDIS Planner to include Coordination of Supports from MDQ in your NDIS Plan.
